角化あるいは腺系への分化を示す上皮性成分とspindle cell主体で脂肪の混在した間質成分とからなるwell
circumscribed biphasic tumorで、mitosisは認めません。高齢者の頸部病変との臨床所見とあわせ、Ectopic
hamartomatous thymomaの像と考えます。グロムス腫瘍と考えます。 |

| |
It seems to be ectopic hamartomatous thymoma (branchial
anlage mixed tumor) consisting of epithelial, spindled and adipocytic
components. It is of interest whether the epithelial cells express
CD5 in this setting. Myoepithelial and mucoepidermoid tumors must
be differentiated. |

Dear Black Jack: I am not familiar this entity. Thanks
your expert opinion. I wonder your proposed context about the CD5.
CD5 stains for T cell, T cell lymphoma,mantle cell lymphoma, B cell
chronic lymphocytic leukemia/small lymphocytic lymphoma.
Is the CD5 what you want to say CK5? May you have a nice day. |

Hi! How are you doing? CD5 has been demonstrated on
epithelial cells of type-C thymoma (thymic cancer) and CASTLE, but
not of type-A, type-B thymoma and SETTLE. I just wonder if ectopic
hamartomatous thymoma (EHT) express CD5 or not, because there is no
paper dealing with this issue. In addition, spindle epithelial cells
in medullary thymoma have been shown to express CD20. If EHT express
either CD5 or CD20, then it will be a thymic neoplasm. The thymic
differentiation of EHT has not been confirmed, despite of its name
of "thymoma". |
