My diagnosis is Melanocytic nevus. I can see eosinophilic intracytoplasmic inclusion (Gottlieb body) in balloon cell.

  Case 180: Nevus cells
 

多核細胞を形成している部分、脂肪に置き換わりつつあるように見える部分があります。色素性母斑の脂肪変性がこのように見えるのでしょうか。

一部にballon cell changeをみとめます。この変化がどれくらい優位になったらballoon cell nevusと診断するのでしょうか?

"A few large, vacuolated nevus cells (balloon cells) are sometimes found in ordinary nevi. These may be mononuclear or multinucleated. When such cells constitute a significant part of the nevus (arbitrarily 50%), the nevus is designated a balloon cell nevus."という記載がDr WeemsのWebSiteにあります。

What is the biological difference between foamy and ballooned nevus cells? If it is merely the difference in size of cytoplasmic vacuoles, such a distinction might have little meaning.

You will see Balloon cells in Melanocytic nevi and Malignant melanoma. You won't see foam cell with pink globules in Malignant melanoma.
This observation is so far personal experience but usefull in practice as a clue in differntial diagnosis.
