真皮間質の高度edema と急性炎症に、毛包を中心とする多核巨細胞を伴う肉芽腫性炎症を認め、一部では丸い菌体様構造物を認めます。スポロトリコーシスの像と考えますが、PAS染色などの特殊染色が必要です。

There is an ulcerated skin lesion assiciated with diffuse suppurative and focal granulomatous reactions in the dermis. There is also something stained faintly blue in the clear tissue spaces that might be a pathogen responsible for this disease. Sporotrichosis would be a good answer, but special staining should definitely be required for the final diagnosis. Ruptured deep folliculitis might give the similar result as well.
