It is an ill-defined fibrotic spindle cell lesion
with focal pigmented areas and mild epidermal atrophy, suggesting
dermatofibroma (DF) rather than blue nevus. If this is correct,
the pigment must be hemosiderin but not melanin, as dendritic melanocytes
can merge with DFSP but not with DF. Some of pigmented cells appear
spindle or dendritic, which, however, does not necessarily mean
that they are melanocytes, since dermal dendrocytes capturing pigment
may also give the similar morphology. Routine histochemistry would
be appreciated to delineate the nature of pigment, being either
hemochrome or melanin. Immunostaining seems unnecessary. |
